To access your own medical record, please complete the application form and send a copy of your ID to nsp.onlineservices@nhs.net

The application form advises us of what information you would like access to.  Once we recieve your application it will be processed and the required codes will be sent to your registered email address.  You will initially be give access to Allergies, repeat medication and immunisations.  If you have requested full access this needs to be authorised by your GP and can take up to 28 days.
