Everyone must stay at home to help stop the spread of coronavirus.
You should only leave the house for very limited purposes:
Important - These reasons are exceptions – even when doing these activities, you should be minimising time spent outside of the home and ensuring you are 2 metres apart from anyone outside of your household.
There is separate advice about staying at home if:
Do not leave your home if you have either:
To protect others, do not go to places like a GP surgery, pharmacy or hospital. Stay at home.
Use the 111 online coronavirus service to find out what to do.
Only call 111 if you cannot get help online.
Important - Call 111 for advice if you're worried about a baby or child.
If they seem very unwell, are getting worse or you think there's something seriously wrong, call 999.
Do not delay getting help if you're worried. Trust your instincts.
Use the quick NHS coronavirus status checker to tell us about your current experience of the virus.
This will help the NHS plan its response to coronavirus by showing where the virus is spreading and how it affects people.

‘Morning After’ contraception works best when taken within 24 hours although it is still effective up to 72 hours after sexual intercourse. You can book to see a practice nurse or a doctor if you need emergency contraception. It will help you to get an emergency appointment if you can tell the receptionist why you need the appointment. The ‘morning after’ pill is also available over the counter at the chemist.
Thinking of having a baby?
Pre-conceptual advice is available from the Practice Midwife or from your own doctor. Please ask the receptionist to make an appointment.
All the following Clinics take place within the Health Centre:

If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.
There is further information about countries and vaccinations required on the links below:
It is important to make the initial appointment as early as possible. Please complete the form and return to us at least 4 - 6 weeks before you are scheduled to travel for short stays abroad and 12 weeks before long trips. The vaccines have to be ordered as they are not normally a stock item.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
Travel Health Questionnaire
Please complete and return a copy of our Travel Questionnaire to the practice or email to email@example.com. Please give us at least six weeks BEFORE you travel. You can either obtain a form from reception or print one out by clicking on the link here

Private medical examinations for Insurance, Heavy Goods Vehicle, Public Service Vehicle, Hackney Carriage, aviation and parachuting may be arranged with the Doctors by appointment. In addition Dr Partner is qualified to carry out Aviation medicals.
In order to make an appointment for a medical, you need to let the Practice have the papers at the time you make the booking. We are unfortunately unable to carry out a medical until we have received the full details of what is required from the party involved. These appointments usually take 30 minutes and are carried out Tuesday to Fridays.
A list of fees for non-NHS services (eg. medicals and Insurance forms), is available from reception.
Please find below a downloadable BP form. Please do your BP's at home for 1 week and return to firstname.lastname@example.org or hand in at reception desk. Please don't forget to add your name and date of birth. You will only be contacted by a GP if they need to discuss your readings with you
