To register for access to another person's medical record as a proxy user, please click here to complete the online application form and email nsp.onlineservices@nhs.net with a copy of your ID.

To register using the NHS app you will need to download the app from the app store or play store.  You will be required to verify your ID upon registration however to fully access your medical record you will still be required to complete the application forms.
