Dear NSP Patients,
We are launching our Care Navigation Toolkit today which is a signposting tool that our Patient Co-ordinator Team have been trained to use for all patient contacts with the Practice.
You may be asked a few more questions than previously, but this is to help our Patient Co-ordinator Team better understand the help you are looking for, and so they can offer you the most appropriate advice.
As you may know, there are several options for people to access health advice outside the practice such as local pharmacies, the NHS website or self referral services ie NHS Talking Therapies or Achieve Oxfordshire Weight Loss service.
Within Newbury Street Practice, as well as our GPs & Nursing Team, we have many other health professionals including an Advanced Nurse Practitioner, Advanced Care Practitioner, Physician's Associate, Clinical Pharmacists & Pharmacy Technicians, First Contact Physiotherapists, Mental Health Practitioner, Social Prescribers including a Young Person's Mental Health Worker, Health & Wellbeing Coach and Care Co-ordinators.
This means that sometimes you may not be offered a GP appointment but instead, if appropriate you may be directed to seek advice outside the practice, or offered an appointment in the practice with an alternative team member.
We hope that improved Care Navigation will mean we can offer more GP appointment capacity throughout the week for those patients whose medical issues are most suitable for GP care.
